Pharmacology nursing NCLEX review question on bronchodilators and corticosteroid administration for a patient with asthma. This NCLEX pharmacology review question will test your nursing knowledge about how to properly administer a bronchodilator and corticosteroid metered-dose inhaler. To answer this question correctly, you must know the mechanism of action of both the bronchodilator (Albuterol) and corticosteroid (Fluticasone). What is Albuterol? It is a short-acting bronchodilator. A bronchodilator (just as its name says) opens the bronchi and bronchioles of the lungs. When this happens more air is able to flow through the lungs. This is vital for the patient who has asthma because they experience bronchoconstriction. What is a Fluticasone? It is a corticosteroid used to prevent symptoms in patients with asthma. It will work by decreasing inflammation in the small airways. To administer these medications, the bronchodilator will be administered FIRST and then 5 minutes later the corticosteroids will be administered. The bronchodilator will open the airway first so when the corticosteroids are inhaled they can do their job within the lungs. It is important to remember that AFTER administering inhaled corticosteroids, the patient must gargle and rinse the mouth with water and spit (or a spacer can be used). The reason for this is because corticosteroids can cause thrush.
